VOLUNTEERS’ WAIVER CONFIRMATION:
- I am physically fit to participate;
- Will follow all instructions from the MDLA Committee of Management;
- And agree to wear issued MDLA uniforms and comply with behaviour standards.
I understand the physical risks involved in carrying the Dragon components/equipment and, to the fullest extent permitted by law, release and indemnify the Melbourne Dai Loong Association Inc, its Committee of Management Members and Volunteers from liability arising from my participation, except where caused by negligence.
For participants under 18 years of age, parent/guardian consent and supervision is required.
